Important things to know before completing an application

Please be aware you will be required to provide proof of household income once completing your application. Please have this ready via electronic attachments via a PDF or via picture images.

An assessment for Free School Meals will be completed using the information provided on this application form.

Examples of household income evidence we request is shown below:

  • Three most recent wage slips
  • Tax Credit Award letter for the 2022/23 year
  • Most recent P60
  • Three most recent Universal Credit payment schedules which includes name and address, Payment for that month, standard allowance and any child benefit or earned income
  • Three months’ worth of bank statements which show either of the following benefit; Income Support, Employment Support Allowance Or Job Seekers allowance
